Excessive Sleepiness After Stroke
Sleepiness aids brain recovery after stroke, but excessive sleepiness may signal sleep apnea, depression or new problems needing treatment.

Tiredness and sleepiness throughout the day may be observed in stroke patients. Sleeping supports recovery in the brain after stroke. Stroke patients should get enough sleep to process the exercises they have done throughout the day into their brain. However, excessive sleepiness may point to more serious underlying problems.
Benefits of Good Sleep after Stroke
Sleepiness is a normal part of the early recovery period after stroke. In this period, the brain dedicates most of its resources to recovering from the damage it sustained. Therefore, the brain has less energy to keep the person awake.
Deep sleep helps the brain recover in persons who had a stroke. REM Sleep, which occurs at intervals throughout the night, helps the brain process motor information. During this stage, the brain converts short-term memories about muscle movement into long-term information.
Good sleep can help regain movement and muscle strength in stroke patients. Therefore, stroke patients should get enough sleep.
Causes of Excessive Sleep After Stroke
While sleepiness decreases after a couple of weeks in most cases, some patients may experience it for more than six months. If excessive sleepiness prevents participation in rehabilitation and lasts more than a couple of weeks, a doctor should be consulted.
In some stroke patients, the sleep-wakefulness balance may be suddenly disturbed. Relatives should be careful about sudden excessive sleepiness, as it may indicate new brain problems.
Causes of excessive sleepiness after stroke include:
Sleep-Disordered Breathing (Sleep Apnea): A chronic condition causing partial or complete breathing cessation during night, resulting in daytime excessive sleepiness.
Reverse Robin Hood Syndrome: Excessive sleepiness occurring when the brain channels blood flow from damaged areas to undamaged areas.
Depression: Severe depression can cause extreme fatigue and drowsiness.
Symptoms of Excessive Sleepiness After Stroke
The first symptom is feeling tired throughout the day. The stroke patient with excessive sleepiness feels tired and sleepy even if they just wake up from a full night's sleep. Other symptoms include:
Angriness
Apprehension
Decreased appetite
Cognitive disorders
Slow speech
Memory problems
Restlessness
Treatment of Excessive Sleepiness After Stroke
Experiencing excessive sleepiness may be natural for stroke patients in the early period as part of the recovery process. However, if sleepiness prevents participation in therapies, it negatively affects recovery.
Treatment approaches include detection and treatment of underlying causes, doctor-prescribed medications to decrease sleepiness, and dietary changes.
The first step involves identifying underlying reasons. For example, sleep apnea may require a sleep device ensuring adequate oxygen. If depression causes excessive sleepiness, post-stroke emotional changes can be managed through medication and psychological counseling.
